Transformational Change Programme

The Joint Community Care Plan sets out the high-level outcomes it seeks to achieve (see The outcomes we seek). To achieve these it is recognised that we must deliver on a number of improvement objectives (see Areas for change and improvement).

For these improvement objectives to be achievable and sustainable, there needs to be a complementary and underpinning programme of change that addresses and supports transformational change in systems and business processes.

This major transformational change programme is being supported, both in terms of developmental and implementation activities, by a dedicated support team, which links closely with managers and practitioners across the new integrated health and social care service, and the voluntary and private sectors.

The Change Support Team comprises managers and practitioners in adult care services. Its work is focussed on progressing ten separate, but inter-related, workstreams. Information on these workstreams is available on the right-hand side of this page.
