The Change Programme

The Highland Community Care Partnership is setting out the high-level outcomes it seeks to achieve with Users and Carers (see The outcomes we seek). To achieve these it is recognised that we must deliver on a number of improvement objectives (see Areas for change and improvement).

However, for these improvement objectives to be achievable and sustainable, we believe there needs to be a complementary and underpinning change plan that addresses and supports transformational change in system and business processes.

Such a change plan will require ongoing support, both in terms of developmental and implementation activities, from a dedicated support team, which links closely with managers and practitioners across the Highland Council, NHS Highland and independent sector.

A Change Support Team has now been formed, involving managers and practitioners in community care services from both Highland Council and NHS Highland. It is focussed on progressing 15 workstreams to support change. A summary of progress against the workstreams is available on the right-hand side of this page.
